- Understand the Purpose of TRT Therapy:
- TRT is a medical treatment prescribed exclusively to address conditions associated with clinically verified testosterone deficiency. The treatment is tailored to your individual needs following a comprehensive medical evaluation, including blood work and a detailed consultation.
- Provide Accurate Information:
- You certify that all personal and medical information submitted to TRT Australia is accurate, truthful, and complete. Misrepresentation or omission of material facts may result in termination of services and potential legal consequences.
- Consent to Data Storage and Usage:
- You grant TRT Australia permission to securely store and process your medical data solely for evaluating your eligibility and providing TRT treatment. Your data will never be shared with third parties and will be managed in compliance with AHPRA, TGA, and Pharmacy Board of Australia standards.
- Acknowledge the Risks and Benefits:
- You confirm that you have been informed of and understand the potential risks and benefits of TRT therapy. These include but are not limited to hormone level adjustments, possible side effects, and the necessity for regular monitoring. Full disclosure of risks and benefits will be provided during your consultation.
- Agree to Consultation and Verbal Consent:
- By participating in the phone consultation and verbally confirming your agreement, you provide valid informed consent for TRT Australia to initiate and manage your therapy. This includes understanding that the therapy will only be prescribed following an appropriate medical assessment.

This Agreement strictly adheres to Australian laws and guidelines, including:

Confirmation of Consent
By submitting your medical data, participating in a phone consultation, and verbally confirming your agreement with a TRT Australia representative, you provide valid informed consent to begin treatment. No additional signature is required. Your actions affirm that:
- You have read and understood the Terms and Conditions and this Informed Consent Agreement.
- You consent to TRT Australia securely storing and using your medical data solely for the purpose of managing your TRT therapy.
- You agree to the commencement of treatment based on the medical advice provided during your consultation.
